RAFT Dementia Support Program Eligibility and Referrals

Eligibility Requirements

• Resident of the City of Alexandria, Arlington, Fairfax-Falls Church, Loudoun, or Prince William Counties
• 55 years of age or older
• Diagnosis of dementia or exhibiting symptoms of dementia with behaviors
• Living in the community in their homes, or the homes of family members or other caregivers

How to Make a Referral

Anyone can make a referral to request education and training from the RAFT Dementia Support Program. Referrals can come from individual family members, government agencies such as APS, AAA, and CSB, as well as community agencies who work with older adults in the community such as home health care agencies.
